It is not because the service disappointed them. It is not because they changed their mind. Something in the gap between intention and action broke down, and you have no idea where.

Most advice on collecting reviews focuses on the ask itself - when to send the request, what words to use, how to phrase it. That's useful. But it assumes the main barrier is getting the customer to want to leave a review. For happy customers, that's often already true. The real problem is what happens after they say yes.
